Made 4 Math Monday - Student Information Form

It's late ... first Monday was fun ... and challenging!

The one thing I made for this first week is a Google Form ... to collect student information and parent contact info.    Here is what a hard-copy would look like:

When students complete the form, the information is collected in a google spreadsheet which gives me a ready-made database for contact information.
